At Private Care & Continuity Planning, our work has always been rooted in one guiding principle: continuity is the ultimate expression of care. It is not enough to address immediate needs—we exist to ensure long-term stability, dignity, and peace of mind for individuals and families navigating complex care decisions.

Based in Calabasas and Hidden Hills, California, our practice serves clients nationwide, providing a discreet, high-touch care model designed for families who value thoughtful coordination and long-term planning—no matter where they are located.

As our practice continues to grow, we are now thoughtfully accepting a limited number of new private-pay clients.

A Personalized, Long-Term Approach to Care

We serve individuals and families who are seeking more than fragmented services or short-term solutions. Our model is intentionally comprehensive, designed to support people across every stage of life and care.

Our services include:

  • Psychological evaluations and clinical insight

  • Coordination of clinical therapies and professional care teams

  • Private caregiving and professional staffing support

  • Family education, advocacy, and systems navigation

  • Long-term care continuity planning focused on stability, protection, and future readiness

We frequently support families of individuals with autism spectrum disorder and related learning differences, as well as seniors and veterans—always with discretion, intention, and respect for the whole family system.

What Makes Our Practice Different

Private Care & Continuity Planning is not a high-volume model. We do not rush care, outsource oversight, or take a transactional approach to deeply personal decisions.

Instead, clients receive:

  • Direct access to a dedicated care strategist

  • Thoughtful coordination across clinical, care, and continuity planning domains

  • Clear communication, professional boundaries, and ethical oversight

  • A long-term vision for care that evolves as needs change

This structure allows us to remain deeply involved, proactive, and aligned with each client’s goals—locally and nationwide.

Who We Are Currently Welcoming

We are currently accepting new clients who:

  • Value comprehensive, coordinated care

  • Prefer a private, concierge-level approach

  • Are seeking long-term continuity rather than crisis-based solutions

  • Understand the importance of planning ahead—not reacting later

All services are provided on a private-pay basis, allowing us to maintain the highest level of discretion, flexibility, and quality of care for each family we serve.

Beginning the Process

New client engagements begin with a confidential consultation to determine alignment, scope of need, and the most appropriate path forward. This ensures that every client we accept receives the level of attention and stewardship our practice is built upon.

If you are seeking a thoughtful partner—rooted in Calabasas and Hidden Hills, yet equipped to serve families across the United States—we welcome the opportunity to connect.
